    This is from memory, but someone will correct me if I get it wrong. Extra attacks occur once your BaB (Base Attack Bonus) reaches +6. For the fighter classes (fighter, barbarian, monk, paladin), BaB goes up +1 per level. less so for the other classes. At +6, you get a seond attack at +1. This scale continues when the 2nd attack gets to +6, so then you would have a 3rd attack at +1. Would look like this: +11/+6/+1 and so on.

    Monks have a special ability called flurry of blows that they get at 2nd level (I think). It allows them to make another attack at their base BaB, with a penaly to both attacks of -2. So, at 2nd level, their BaB would be +2, meaning they could turn on flurry of blows and get 2 attacks at +0/+0, instead of 1 attack at +2. This penalty is reduced by 1 per level (if I remember correctly), so at 4th level they don't have a penalty and at 5th level they start getting boni to hit (I think). Hope that helps.
