The ogre caves are only accessable if you do the fire temple quest 2 (you can botch the first one if needed and still get there), the islands? perhaps you mean the elemental nodes? They are accessable from Hedrack's room (level 4 of the temple) but getting out is pretty hard - make sure your a high level if you plan to explore there, and watch out for the game killing lag.
