Losing attacks and actions apparently with no reason

    It sounds like the problem may be the hard-coded limit of number of combatants. I forget the number but if it exceeds it, then often it will skip some actions because of this limitation.

    I can't remember what triggers the overload skip for a combatant. It might be based on initiative order or particular action or even range from the center of conflict. o_O

    Common big battles where this may occur are Lereth, monster mash, giants, etc. If you limit the scope of your battles, it can be often avoided.

    But if it is simply referring to losing an action without exceeding combat limits, then

    ...sometimes (rarely) you can empty your action bar without doing what you want - and it is usually something to do with a hybrid action. Iirc, this is almost always avoided by using a full action selection or something that uses up the full action (charge, run, etc.).

    Unfortunately many pure-attack full-actions are a bit of a waste until you get more than one attack. So ymmv with this and it is usually worth the rare pain to do hybrid stuff.

    In this case, I think the whole turn is skipped. You do not get the blank action situation.
