It's a magical formulae set in the game engine that takes into account weapon, class, bonuses, armor class, etc. Then the random number generator decides if you hit or not. It takes into account both the attacker and the opponent and the revelant skills, levels, HD etc. Basically, the DMG on computer.

There are many elements. The best way to find out what affects your rolls in your time it to click the result in the roll history. You will find all of them listened and summarized.

Melee Attack Total (using melee weapon(s)) = 1d20 + (Base Attack Bonus) + (Strength modifier) + (weapon bonus) +/- (miscellaneous modifiers)

Exception: With the Weapon Finesse feat with a light melee weapon or a rapier made for a creature of your size category, you may use your Dexterity modifier instead of your Strength modifier on attack rolls. If you carry a shield, its ACP (Armor Check Penalty) applies to your attack rolls.

Ranged Attack Total (using a bow or crossbow or sling) = 1d20 + (Base Attack Bonus) + (Dexterity modifier) + (weapon bonus) +/- (miscellaneous modifiers)

Your Attack Total has to be greater than or equal to your opponent's AC (Armor Class) in order for you to hit your opponent.

    Exceptions:

A natural attack roll of 20 always hits.

A natural attack roll of 1 always misses.

    Note:

BAB (Base Attack Bonus) depends on your class(es) and level(s).

Fighters, Paladins, Rangers and Barbarians have the best BAB (Base Attack Bonus). Wizards and Sorcerers have the worst BAB.

    Melee-only attack modifiers:
    +2 Charging
    -# Two-Weapon Fighting
-1 Buckler equipped while using 2-handed or off-hand weapon
    +# Belt of Giant Strength
+1 Gauntlets of Ogre Power (bonus does not stack with Belt of Giant Strength or Pale Blue Ioun Stone)
+1 Pale Blue Ioun Stone (bonus does not stack with Belt of Giant Strength or Gauntlets of Ogre Power)

    Ranged-only attack modifiers
    +1 Point Blank Shot (feat)
    -4 Firing Into Melee
    -2 Rapid Shot (feat)
    +1 Lesser Bracers of Archery
    +2 Greater Bracers of Archery
    +# Gloves of Dexterity
+1 Deep Red Ioun Stone (bonus does not stack with Gloves of Dexterity)

    General attack modifiers:
    +1 Weapon Focus (feat)
+1 Greater Weapon Focus (feat) (Fighters only)
    +2 Flanking
    -5 Second Attack
    -10 Third Attack
    -15 Fourth Attack
    +4 Target Prone
    +4 Target Helpless
-1 Dodge (feat) (opponent has this feat)
    +2 Attacker Not Visible
    +#/-# Size Adjustment
    +1 Prayer (spell)
    -1 Bane (spell)
    +1 Haste (spell)
    +1 Bless (spell)
    -2 Fear (spell)
-4 Cover (i.e. somebody else is between you and your target)
    -4 Weapon Nonproficiency Penalty
-1 Temporary Level while weapon (or Hedrack's Shield) that has an opposite alignment to your character is equipped

The main things that effect your chance to hit are:

-Your BAB, which depends on your class and increases sometimes when you level

-Your Strength Modifier, which increases the damage you do, too

-The weapon's +'s, which also increase the damage you do

-The rest are are modifiers due to circumstances like flanking, cover, target movement, etc

-Your Dexterity Modifier effects your To Hit chance when you have the Weapon Finesse Feat and the weapon is a light one or a rapier

You also get +2 to hit when you are invisible if I'm not mistaken.
